- Address: building number and name of the communication route corresponding to the physical location of the company that carried out the work in situ.
- City: city in which the company that carried out the work in situ is located.
- Postal code: sequence of numbers and letters in the form L9L 9L9 that corresponds to the address.
- Telephone number: telephone number used to contact the company that carried out the work in situ.
- Type of treatment: indicates the type or types of treatments that have been used by the company and authorized by the Ministry in the last five years.
